We accept the following methods of payment:

How to Order Online

Here is how to place an online order for items in our store: 

1. Browse the available products and make your selections by filling in the quantity box next to the items you want to order. 

2. Complete the information boxes at the bottom of the order page, including payment information. Online orders may be paid by Visa or MasterCard only.

3. Click on the "Submit" button.

You will receive a confirmation via e-mail within about 48 hours (business days) showing your total purchase price including tax and shipping and handling.

How to Order By Mail or Fax

Here is how to place an order for items via mail or fax:

1. Print our special mail / fax order form.

2. Indicate your selections on the order form by checking the box next to the item and filling in the quantity. 

3. Calculate the total where indicated. Washington residents add 8.6% tax where indicated.

4. Complete the billing, shipping, and payment information boxes.

5. Mail or fax your completed form to the address or fax number indicated on the form.

Mail orders may pay by Visa, MasterCard, Check, or Money Order. Fax orders may be paid by Visa or MasterCard only.

You will receive a message via e-mail within about 48 hours (business days) of our receipt of your order confirming your total purchase price including tax and shipping and handling.

Shipping Information

Center for Life Decisions charges for shipping on a  PER PRODUCT basis. The shipping and handling charges are listed next to each product on the products page. Rates are for ground shipping by UPS or US Postal Service to US addresses.  For other delivery addresses, we will contact you via e-mail or phone with rate information, before processing your order. 

Payment Security

If you use a browser that supports secure connections, your order will be handled using our secure payment system that protects your credit card information while in transit over the Internet.
